The Community Care Plan of Eastern North Carolina is an innovative program that aims to help persons who are on Carolina Access Medicaid to prevent complications from chronic diseases, avoid costly emergency room visits, and unnecessary hospital stays.

If you are a Carolina Access Medicaid client and under the care of a participating physician, you are automatically eligible for services provided by  Registered Nurse Case Managers for Carolina Access Medicaid. Their office is located at Carteret County Health Department and can be reached by calling 252-728-8550.  They can assist you with:
  • Understanding and learning about your medical condition
  • Schedule appointments with you doctor
  • Attend doctor visits with you to help answer questions
  • Assist to arrange transportation to doctor appointments
  • Help you get your medicine and any equipment you may need
  • Visit in your home for teaching about your medical condition
