Communit Paramedicine is a program that offers county residents support in order to better manage illnesses in the home. The program helps patients manage chronic diseases such as diabetes, high blood pressure, congestive heart failure, or COPD. Home checks are completed in order to verify compliance with prescription medications, assess blood pressure or blood sugar, and to confirm the safety of the environment. The program provides home education on new or existing conditions and appropriate follow-up care for high-risk patients in order to reduce the hospital readmission rate.
