Obtaining Your Medical Records at Carteret County Health Department

How to Request Your Records

You can request your medical records in two ways:

• In Person: Visit the Medical Records Division at the Carteret County Health Department located at 3820 Bridges Street, Suite A, Morehead City, NC.
• By Phone: Contact the Medical Records Department at 252-728-8550 for detailed instructions.

Requirements

• Valid Identification: You will be required to show a current photo ID before receiving or viewing your medical records. If you don't have a photo ID, you must answer security questions to verify your identity.
• Authorization Form: Complete an Access for Records Request form (available in English and Spanish).
• Fee: There is a $15 fee per record.
